What do I need for completing the form?

You should read our guidance and pavement licence procedure document before making your application.

You will need to upload the following documents as part of this form:

• A site plan of where the proposed activity will take place
• A copy of the certificate of insurance covering third party and public liability risks to the level of £5 million
